Applications & Forms

To register, download and fill out all the following forms and bring them to the coach at the Trousdell Aquatics Center between 3:00pm and 6:00pm, Monday, Wednesday or Friday. Swimmers will be swim tested on site at time of registration. Please bring a swimsuit, towel, and goggles if desired. Parents will also need to bring a copy of the child's birth certificate, their checkbook and their drivers license.

2009 Registration form to join US Swimming. Needs to be renewed each year
